I understand that my child will be exposed to a variety of foods. I understand my child will be working with cooking tools with supervision. I understand the nature of the proposed activities and hereby assume any and all risks associated with those activities.

Release, Waiver of Liability and Indemnification

I, on behalf of my child, hereby release and waive any claim or prospective claim of liability by my child or by me on behalf of my child, against GOURMET KIDS PARTY (Rosing Enterprises LLC), their employees and agents, with respect to any injuries, illness, damage or death occurring to my child while he or she participates in any and all Gourmet Kids program activities.

I do hereby consent and agree that Gourmet Kids has the right to take photographs and video of my child during the program sessions. These photos and video may be used on the company website (www.gourmetkidsparty.com), company Social Media pages, and promotional material without compensation. I understand that my child’s name and identity will not be revealed. If you would prefer we not take photographs or video your child, please notify us via e-mail.

My child has my permission to participate in GOURMET KIDS. By signing below I release any claims, damages and liabilities arising from or related to my child's participation in this program.
